General Forms

  • Change Meal Plan
    Meal plans CANNOT be changed after the third week of classes each semester. Students must log in to StarRez via MyUTampa.
  • Incident Report Form
    Complete this form to report an incident occurring in the residence halls.

Housing Information and Forms

Most housing forms can be found in StarRez. Students can access StarRez through the chiclet in MyUTampa.

  • Fall 2025/Spring 2026 Housing Agreement (PDF)
  • New Student Housing:
    Information is available online in your SpartanStart portal. New students should submit their housing application in StarRez  after deciding to attend UTampa and making the required deposit.
  • Returning Resident Room Selection
    Only current residential students (including those studying abroad or interning through UTampa) may participate in this process.
  • Summer Housing Information
    For more information about summer housing, please visit the page above.
  • Summer Housing Application
    This form is for students attending classes in the summer and in need of on-campus summer housing. Students must log in to StarRez via MyUTampa to access the application.
  • Winter Break Housing
    This form is for current residential students in need of on-campus housing over the winter break. Students must log in to StarRez via MyUTampa to access the application.
  • Residential Student Below Full-time Status Request
    If a student has dropped below full-time student status and wants to remain in the residence halls, they must complete this form for approval. Students must log in to StarRez via MyUTampa to access the application.
  • Standby List
    This form is for continuing students. Vacancies will be filled from this list only if spaces are available or become available at a later date. There is no guarantee that students will receive an assignment on campus by being on this list. Students must log in to StarRez via MyUTampa to access the application.
