This form is to be used by visitors only and submitted at least one business day prior to your expected arrival on campus. The form must be filled out completely. If there is a section that does not apply then use N/A; this does not include vehicles. All vehicle information must be included or you will not receive a pass. Students, faculty and staff must contact Campus Safety if they need a temporary pass. If you attempt to use this form, you will be reported to conduct (students) or your supervisor and appropriate senior staff (faculty and staff) will be notified.
For information on available parking areas, please review the visitor's section on our parking information page. If you have not received an email with your temporary pass by the time you visit the campus, you must come into the Campus Safety Office to pick one up.
